Practice Guidelines and Other Documents for Review

In order for our practice to provide you with the best care, it is essential that you are fully informed regarding the midwives model of care and scope of practice, what our training involves, as well as being completely informed about the risks and benefits of care with a midwife and planning for a safe homebirth.  These documents offer you this information in a concise format.  I am required to provide this information to you prior to initiation of care.  Clients are required to sign a consent form before care is initiated indicating that they have had appropriate access to these documents and that they are informed about the health care choices they are making as well as their providers at Appalachian Birth Co-Op and Wiyama Midwifery.  The CV about each member of the Wiyama practice can be found on the About Us page.
